But that officially started changing in 2026.
As of January 1, 2026, Traditional Medicare began requiring prior authorization for certain outpatient procedures in six states, including Texas, under a new Medicare pilot program.
That means some services may now require:
- Advance approval before treatment

- Additional provider paperwork
- Potential delays for select procedures
The program currently focuses on 17 outpatient services and is expected to continue through 2031.
